This course reviews the Individual Assessment Form.
The Individual Assessment Form (IAF) is used to determine how much direct support a person who lives in an intermediate care facility needs from care staff, including nurses. Certified ICF assessors who are familiar with a person’s daily needs will assess their personal care abilities, and behavioral and medical needs.
The IAF training certifies someone to perform the IAF assessment.

This course reviews the Level of Care assessment.
The Level of Care assessment is used to determine whether a person has a developmental disabilities level of care. Establishing the level of care a person may need is necessary for admission to an intermediate care facility or for enrollment in a home and community-based services waiver. The LOC is not intended to be a comprehensive assessment of the person. It serves as a foundation for comprehensive assessment and service planning.
The final LOC test is only available to people with certain roles in the DODD portal.

This online course reviews the Ohio Developmental Disabilities Profile assessment.
The Developmental Disabilities Profile is an assessment required for people accessing the Individual Options Waiver and for residents of an intermediate care facility. The assessment must take place after enrollment in a waiver or admission to a facility but before service planning is complete. The DDP asks many detailed questions to try to understand a person’s needs and circumstances.
The final test of the ODDP course is only available to people with certain roles in the DODD portal.
